Application Instructions for Former Mizzou Graduate Students

• Students who have previously attended the University of Missouri as graduate students more than one year ago must submit a Request to Re-enroll form (PDF), also available from the Graduate School, 210 Jesse Hall, Columbia, MO 65211, 573-882-6312. Send the form to the degree program to which you wish to re-enroll, unless you are a post-baccalaureate (non-degree) student returning for additional studies. In this case, send the form to Graduate Admissions, 210 Jesse Hall, Columbia, MO 65211.
• Complete and submit any required academic program application form/process to the program’s office. Consult with the academic program of interest to learn the deadline for applying or being readmitted to its graduate degree program.
• Official transcripts or mark sheets for any work done since you were last enrolled in the degree program must be sent directly from each university or college you have attended. The documents should be sent directly to your former degree program.
• Submit any requested test results (if not already on record) to the academic program.

Note: Former MU undergraduate students must submit the regular Graduate School application form along with the application fee and other supporting materials.
