The University of Missouri requires all new students to submit an MU Immunization/TB Screening Requirements form and verification of TB screening before registering for classes. The completed form must be submitted directly to the MU Student Health Center. You will also be required to submit evidence of immunization in the form of a copy of one of the following:
- Personal immunization records
- Physician immunization records
- School immunization records
