Certification Form Instructions

Points 1 through 7 are to be filled out by the student's employing department. Departmental fellowships or scholarships need to be filled out by the host department.

  1. Please check the semesters in which your department will employ the student (the dates should match the Personnel Action Form (PAF) and the appointment dates that are listed under assistantship information on this form). If you do not know if the student will be employed in the second semester, do not check the box. Send another form later when a new PAF is completed.
  2. Student Name (Last name first) should match the name on the PAF. Make sure that the first name is the same because some students use their middle or nicknames. If a student changes his or her last name in the middle of the semester, please notify us as soon as possible.
  3. We must have a student number! If you do not provide us with a student number, we will not be able to enter a waiver. Make sure the student number is the most recent one because some students may have changed their student number. If the student is new, please make sure he or she has a student number before completing this form.
  4. Please provide the 9-digit social security number. Make sure that it matches the PAF. If the student is new and does not have a SSN, he or she will need to get one to remain eligible for the fee waiver.
  5. Academic Degree Home (not college) should be the same as in the student information system. We do not need to know the area of study. For example, Music Education is not an academic degree; rather it is a part of Curriculum & Instruction.
  6. Please provide the name and phone number of the person who is completing the form for the student and knows the most about the assistantship and the fee waiver. This should not be the student's name and phone number, but rather someone from the employing department.
  7. Assistantship or Fellowship:
    1. If the student has an assistantship, you need to provide all of the following information:
      Title
      In order to qualify for a waiver, a student must have one of the titles designated by the Graduate Faculty Senate (title code in the brackets must be the same on the PAF).
      Appointment Dates
      Dates should match the semester(s) you checked for part 1 and be the same on the PAF. If the appointment is not for the whole semester, a partial fee waiver may be given. Partial fee waivers will be pro-rated by the number of working days the student has worked (or will be working) compared to the number of total working days in the semester.
      Amount
      Please write the amount the student will be paid for the total appointment duration. The minimum required stipend amount for 2008-2009 is $2,452.00 per semester for a quarter-time appointment, or $545.00 per month (the assumption with the monthly amount is that Fall is 4.5 months long). If the total amount paid to the student for the semester is less than this amount, then the student is not eligible for a tuition waiver.
      FTE
      Please provide the FTE (full-time equivalent) under which the student will be employed. For example, 10 hours a week is considered 0.25 FTE; 20 hours a week is considered 0.50 FTE. If the student has different FTEs for two semesters, please fill out two different lines, as if the student has two different appointments. The minimum FTE is 0.25.
      Please check if the student is going to be funded from a grant/contract.
      Make sure to complete the Employing Department field on the Certification of Eligibility form and have the appropriate person sign it.
    2. If the student has a departmental fellowship or scholarship, please provide the name of the Fellowship/Scholarship, beginning and ending dates of the Fellowship/Scholarship and the amount of funding provided by the Fellowship/Scholarship, with the minimum amount being no less than $2,150 per semester.
  8. Please have the academic program chair of the student sign at the bottom of the form. The Graduate School has an updated list of all the graduate chairs for each department, and we will verify signatures with that list. If you need to change the name on this list, please contact the Graduate School at 882-9576.
