Medical Requirements: Immunization & TB Screening

New Students Must Submit an Immunization Policy Form

The University of Missouri requires all new students to submit an MU Immunization/TB Screening Requirements formand verification of TB screening before registering for classes. The completed form must be submitted directly to the Student Health Center. You will also be required to submit evidence of immunization by a copy of your:

• personal immunization records (e.g., a "baby book") or
• physician immunization records or
• school immunization records
