Survey of Earned Master’s/ Educational Specialist Degree
The purpose of this survey is to collect demographic data on students completing a master’s or educational specialist degree at the University of Missouri, as well as information on placement and degree satisfaction. Although the survey is now online, you are still asked to fill it out before turning in your thesis. A print-out of the survey is not necessary — just fill it out online, and you’re done.
Your participation in this survey is completely anonymous. The data is only used in summary.

Survey of Earned Doctorates
The purpose of this survey is to collect demographic data on students completing a doctoral degree at the University of Missouri. The survey is conducted on a nationwide scale through six federal agencies including the National Science Foundation and National Institutes for Health. Your personal data remain confidentail. Resulting national reports contain summarized (aggregated) data.

PhD Completion Project Survey
This exit survey is required for students from the following programs: Animal Sciences, Biological Sciences, Chemistry, Civil and Environmental Engineering, Communication, English, History, Molecular Microbiology and Immunology, Neuroscience, Physics and Astronomy, Psychological Science, and Veterinary Biomedical Sciences. Proof of completion of this survey can be provided by printing the final page of the survey and submitting it with your dissertation materials.
Addendum to the Survey of Earned Doctorates
The purpose of this addendum is to collect data from doctoral candidates on post-graduation placement as well as degree satisfaction at the University of Missouri–Columbia. This information is held at the Office of the Graduate School and is distributed only as anonymous data in summary.
Master’s/ Ed. Specialist or Doctoral Processing Fee Statement Form
This form is to be submitted along with a check or money order only. Because the Office of the Graduate School is collecting this money on behalf of another entity, no other method of payment can be accepted.
This fee is associated with the Master’s/Doctoral Agreement form and is used to pay for the processing fee service that archives all theses and dissertations submitted to the University of Missouri.
In addition to the processing service, for an additional fee, students can opt to have this vendor file paperwork on their behalf to copyright their thesis or dissertation.

Original Approval Page
A signed, printed copy of your original Approval page needs to accompany your CD submission.

Electronic Master’s Thesis or Doctoral Dissertation Release Form
This form allows the Graduate School to release your work electronically on the Web. By signing this document, you have also verified that you have obtained permission from the owner(s) of all third-party copyrighted matter.
There are different levels of release available:
- Release the entire work immediately for access worldwide.
- Release the entire work for University of Missouri access only.
- Release the entire work for University of Missouri campus access only.
- Delay the release of the entire work for one year.
The release must be signed by you and your adviser.

Thesis or Dissertation Agreement Form
The purpose of this form is to obtain your signed approval to allow the company that does all of the University’s thesis and dissertation processing, UMI/Proquest, to process your research.
In addition, this form is used to verify that you have obtained the necessary copyright permissions on all third-party work.
Finally, if you choose to pay the optional copyright fee, this company will also file copyright paperwork on your behalf.
Select the "Traditional Publishing" option on page 3 of the agreement form. Please note that if you choose the Open Access Publishing Options on these forms you will have to pay an additional $95 to have your work made available freely on the internet via ProQuest/UMI. All works submitted to the Mizzou Graduate School will automatically be made available on the internet (in compliance with the terms specified in your Release Form) at http://etd.missouri.edu
If you select the "No Search Engine Access" restriction, please make sure your MU Electronic Thesis and Dissertation Release form restricts the work to University of Missouri System or University of Missouri campus only.
